Tri State Orthopedic Institute announces permanent closure

MOHAVE COUNTY, Ariz. – The medical offices of Tri State Orthopedic Institute, LLC in Bullhead City, located at 1225 Hancock Road and the satellite location on Western Ave. in Kingman, AZ has permanently ceased operations. Tri State Orthopedics opened their doors in 1993, formerly located at 2000 Hwy 95, providing comprehensive general and adult reconstructive orthopedic care to the local tri state community.

Due to the extended medical leave of Dr. Robert Lock, a difficult decision was made to close the practice. “I am deeply grateful for the outpouring of support and understanding over the past 6 months during my medical leave. I have been blessed with a long and fulfilling career and have been fortunate to have fostered some wonderful relationships with my colleagues, co-workers, patients and their families,” said Dr. Lock.

Tri State Orthopedic extends their sincere gratitude to the community for three decades of trust in our surgeons and staff to treat their orthopedic needs. Patient’s may obtain copies of their records by emailing their request to tsorecords@yahoo.com or by faxing their request to 928-337-1711 or 928-758-5191. Former patients are encouraged to work with their primary care provider and insurance company to find local options for their continued orthopedic needs.
